A midair explosion of SpaceX’s ( SPACE) Starship rocket in January created a serious and largely undisclosed threat to commercial aircraft flying over the Caribbean, forcing multiple pilots to make emergency decisions while airborne, according to Federal Aviation Administration records reviewed by The Wall Street Journal.
